| 今, 保育所を悩ましているのが保護者からの与薬依頼である.病気の回復期や慢性疾患をもつ子どもにとって, 薬は欠かせない.本来, 保育所では, 子どもへの薬の服用は親が行うことを原則としているが, 働いている親は会社を休むわけにもいかないため, 保育士に与薬を頼むことになる.しかし, 医師でないものが親に代わって薬を飲ませることは医師法による医行為に触れるという法的な問題が絡む.保育所には, 嘱託医を配置することが義務付けられているが, そのほとんどが常駐ではないため, 随時対応できる状況にはない.また, 保育所における看護婦等の配置率は未だ2割弱にすぎず, 保育所の保健対応体制は十分ではない.そのため保育所は, これら保護者のニーズに対しそれぞれ独自の判断で対応しており, 様々な薬の扱いに対する混乱と事故を招いている.一方, 平成12年改訂された保育指針では, 積極的な保健対応策を打ち出した.病気の子どもの保育については, 「乳幼児健康支援一時預かり事業」の活用を推奨すること, 保育中に体調が悪くなった子どもには, 嘱託医などに相談して適切な処置を行うよう特に書き加えるなど, 従来の保育機能に加えて保健対応機能の必要性を示したものと言えよう.そこで本稿では, 保育所が行う与薬を安全かつ適切に行うための与薬行為のあり方とその対応策について, その実態と法的側面, また保育サービスの機能的側面から考察をした.結論として, 保育所の保育士が与薬を子どもにする場合, 医師や看護婦等の協力が条件となること.また, これからの保育所のあり方として福祉サービスと保健・医療は一体的に捉え, 切り離すべきではないことなどである.したがって保育所における保健・医療体制の早期確立が望まれ, 少なくとも嘱託医に加えて看護婦等医療関係者の配置を全保育所に義務づけることがぜひ必要である. | ||||||||

| A problem that is troubling day nurseries today occurs when working parents ask the stafF to give their children medicine. Medication is necessary, of course, for a child who is suffering from a chronic disease or is in convalescence, and day nurseries have used their own judgment in giving medication. However, giving medicine to the children has sometimes led to mistakes and accidents, not only because the Medical Act dose not permit day nursery staff to dose children without a doctor's permission, but because there is seldom a full-time doctor or nurse to consult. Recent, guidelines for childcare institutions have added a new item by which day nurseries are obligated to consult a doctor commissioned for the purpose about the appropriate treatment when a child is taken ill at a nursery. It may be said that these new guidelines show an important improvement in health care in addition to the nursery's functions up to now. To carry out this purpose in the near future, day nurseries are urgently expected to solve certain problems e.g., to find a safe and adequate method of giving children medicine and an improved system of staff of health care welfare. Here a new system of cooperation between the nursery staff and welfare health care staff should be established. For realization of this system, I'd like to propose the idea that a nurse at least should be assigned to day nurseries all over the country. | ||||||||
