現在，介護保険において算定が認められている認知症短期集中リハビリテーションによって，老人保健施設における認知症高齢者の認知機能や意欲，行動・心理症状の改善が報告されている．しかし， 認知症高齢者では同時に摂食嚥下障害を有している高齢者も少なくない．本研究では，認知症短期集中リハビリテーションが，認知症高齢者の認知機能，意欲に加え，摂食嚥下機能に与える影響とその要因を検討した．対象は，老人保健施設において経口摂取を行っている認知症高齢者27名であった．方法としては，約3ヵ月間の認知症短期集中リハビリテーションによる介入を行い，その介入前後で，認知機能のスクリーニング検査である Mini Mental State Examination，HDS-R，前頭葉機能の指標として Frontal Assessment Battery，注意機能の指標として Trail Making Test Part-A，意欲の指標として，やる気スコアならびに Vitality Index と，摂食嚥下機能は，反復唾液嚥下テスト，舌圧の測定，/pa/，/ta/，/ka/ の言語性交互反復運動能力の測定，ならびに Food Intake LEVEL Scale での摂食嚥下状態の判定を実施し変化を検討した．その結果，介入前後で，認知機能は注意機能の Trail Making Test を除き有意な変化は認められなかった．しかし，摂食嚥下機能は/ka/ の交互反復を除き，有意な変化が認められた．老人保健施設入所の認知症高齢者に認知症短期集中リハビリテーションを 実施したところ，注意機能と摂食嚥下機能が改善した．この摂食嚥下機能の改善は，注意機能の改善によって，2次的に随意的な摂食嚥下動作の改善や，全般的な摂食嚥下能力の改善につながった可能性が示唆された． |

Short-term intensive rehabilitation for dementia has been reported to improve cognitive function, vitality, behavioral and psychological symptoms of dementia in older adults with dementia. However, there are not a few older patients with dementia complicated with dysphagia. This study investigated the effects of short intensive rehabilitation for dementia on the cognitive function, vitality and swallowing function of older adults with dementia. Short-term intensive rehabilitation for dementia was conducted for approximately 3 months for older adults with dementia in the care nursing home. Changes in cognitive function, vitality and swallowing function were investigated before and after the intervention. As a result, no significant changes in cognitive function were observed before and after the intervention except for the attention function. Furthermore, there were significant changes in the swallowing function, with some exceptions. These results suggest that dementia short-term intensive rehabilitation for older adults with dementia improved their attention function, leading to improvements in voluntary swallowing movements and overall swallowing ability. |
