| 内容記述 | 上肢ジストニア(書痙,奏楽手痙)に対するボツリヌス毒素療法(Botulinum toxin therapy : BTX)・MAB療法(muscle afferent block)について検討した.2021年9月時点で,針筋電図を用い上肢ジストニア(書痙,奏楽手痙)の患者に対してBTX療法やMAB療法を継続している患者18名(男性9名,女性9名,平均年齢53.3歳,平均投与回数18.9回)について検討した.疾患は,書痙が7例,奏楽手痙が3例,上肢ジストニア(範囲が広いもの)が8例であった.症状を分類すると①母指が屈曲するもの ②手首が掌屈するもの ③手首が背屈するもの ④範囲が広いもの ⑤母指以外の指に限局するもの5種類であった.全例においてBTX療法やMAB療法の有効性が認められ治療が継続されており,有害事象も認められなかった.上肢ジストニア(書痙,奏楽手痙)にはBTX療法やMAB療法は有用であるが,注射部位の選択には注意を要する. | |||||||||

| 内容記述 | We investigated botulinum toxin therapy (BTX) and muscle afferent block (MAB) therapy for upper limb dystonia (writer's cramp and musician's cramp). As of September 2021, 18 patients (nine males and nine females; mean age, 53.3 years; mean number of doses, 18.9) receiving BTX or MAB therapy for upper limb dystonia using needle electromyogram were studied. The diseases included writer's cramp in seven cases, musician's cramp in three cases, and upper limb dystonia (wide range) in eight cases. Symptoms were classified into five types, including: 1) flexion of the thumb, 2) palmar flexion of the wrist, 3) dorsiflexion of the wrist, 4) cramps over a large area of the hand, and 5) those limited to fingers other than the thumb. In all patients, the efficacy of BTX and MAB therapy was confirmed and treatment was continued, and no adverse events were observed. It was found that BTX and MAB therapy are useful for upper limb dystonia, but care should be taken in selecting the injection site of the muscle. | |||||||||
